PrEP: how it works and where to get it

PrEP is medicine people without HIV take to prevent it. Taken as prescribed, it stops HIV from sex almost completely. This page covers how to take it and the three ways to get it, wherever you are.

Daily

One tablet a day, for anyone. Full protection for anal sex after about 7 days; for vaginal sex the CDC says about 21 days and the NHS 7.

On demand (2-1-1)

Two tablets 2–24 hours before sex, one 24 hours after the first dose, one 24 hours after that. Studied in gay and bisexual men having anal sex; not for vaginal sex. Endorsed by WHO and UK guidance, not by the US CDC.

Injectable

Cabotegravir every two months, or lenacapavir every six, given at a clinic. Available in a growing number of countries.

Three ways to get PrEP

A public programme. Free or subsidised in countries such as the UK, France, Australia and Brazil, usually for residents; some clinics accept visitors. Find them in the country list below.
A private or community clinic. Pay per visit and per bottle; prices range from a few dollars a month at Thai community clinics to well over a hundred elsewhere. Listed clinics show what they charge.
Delivered to you. PrEP Online sells generic tenofovir/emtricitabine (the medicine in Truvada) and tenofovir alafenamide/emtricitabine (Descovy) from about US$30 a bottle, shipped to most countries, with tracking. Whichever route you take, have an HIV test before you start and every three months while you take it, and check your country’s rules on importing medicines.

Common questions

Do I need a prescription for PrEP?

In most countries PrEP is a prescription medicine, prescribed after an HIV test and a kidney check. Public programmes and clinics do this in one visit; online sellers ship the medicine and expect you to arrange the tests yourself.

How much does PrEP cost?

From free (public programmes in the UK, France, Australia, Brazil and others, usually for residents) to a few dollars a month at Thai community clinics, and up to several hundred a month at private clinics elsewhere. Online, generic PrEP is sold from about US$30 a bottle.

Can I get PrEP while travelling?

Often, but not always simply: clinics usually want a recent HIV test before supplying it. Bring enough for your trip in its original packaging, and use the country list here to see which clinics start or refill PrEP for visitors.
