We list the government's main sexual health clinic in Colombo, where the national programme says HIV and STI testing is free of charge. It has not said whether that covers foreign visitors, so ask the clinic, and ask about PEP too.

Sri Lanka's National STD/AIDS Control Programme says HIV testing and testing for sexually transmitted infections are provided free of charge at its STD clinics across the island. Its pages do not say whether this covers foreign visitors.

The UK government's travel advice says same-sex sexual activity is illegal in Sri Lanka, and that it is not aware of any prosecutions.
