Sexual health clinics in Japan
We list free, anonymous HIV testing run by city governments and community organisations, which often have English support and give results in person, plus private clinics with English-speaking doctors and the national HIV centre in Tokyo. The free tests state no residency condition but do not mention visitors. Anything beyond them is paid in full without Japanese insurance, and PEP in particular is expensive, though the clinics publish their prices.

Japan's national AIDS Clinical Center says PEP after sex is not an approved use of HIV medicines in Japan, so health insurance does not cover it and you pay the whole cost yourself. At its own Tokyo clinic that is a 20,000 yen first consultation plus 10,000 yen a day for the medicine (280,000 yen for 28 days), before tax. PEP must start within 72 hours. Some private clinics prescribe imported generic medicines for much less: one in Tokyo publishes 66,000 yen for consultation, tests and 28 days of medicine, and one in Osaka 30,660 yen for the medicine alone. We list no PEP provider in Kyoto; Osaka is about 30 minutes away by train.
- Testing
Each city we cover has free, anonymous HIV testing with some English support. In Tokyo, Shinjuku Public Health Center tests on scheduled dates, booked online, and gives results in person only, so it suits visitors who can return. In Osaka, the community centre dista hosts walk-in HIV and syphilis testing for gay and bisexual men several times a year, with results about a week later. Kyoto City gives results of its free anonymous tests only in person, and advises that an HIV test may not be accurate until three months after the possible exposure.
